How to Refer a Patient

 Referring a patient to One Heart Care is simple and designed to ensure continuity and quality of care. Our referral forms are fillable and compatible with most EMR systems for your convenience.

Step 2: Submit Your Referral

Step 3: Patient Contact

Our booking team will contact the patient directly to schedule the appointment(s). Urgent referrals are called within 24 hours, and non-urgent referrals within 72 hours.

Step 4: Appointment Notification

We will fax you a notification informing you of the scheduled appointment(s).

EMR Integration

To support efficient workflows, our referral forms are designed for compatibility with most electronic medical record (EMR) systems. This allows referring providers to easily upload, complete, and share patient information directly within their existing systems, reducing administrative steps and supporting seamless coordination of care.
